We are seeking a full time RN Case Manager to provide clinical care and education to home-based patients receiving home health and/or home infusion therapy.

This position is focused on completing Starts of Care, Recerts & Resumptions of Care (ROC) which will help our agency meet the needs of our patients and referrals sources.

Responsibilities:

• The RN Clinical Manager functions as an office case manager RN supervising field nurses and other team members who are caring for patients in the home setting.
• The RN clinical manager reports to the DON and reviews clinical documentation submitted by field nurses and certified nursing assistants including daily notes, OASIS time points, physician verbal orders, care plans, discharge notes/summaries and assists with ICD-10 coding and plan of care review as needed.
• This position assists the intake coordinator as needed for screening and accepting new referrals for care and staffing cases accepted.
• This position facilitates good communication among the field treatment staff members including nurses, aides, therapists, and social worker as well as office support staff for authorizations and billing issues.
• The RN clinical manager keeps the electronic medical record in compliance with all state and federal regulations in cooperation with the DON, office staff, and the administrator.
• This positions assists in the admin on call schedule to assist with accepting referrals one weekend per month.
• The clinical manager should possess the nursing skills necessary to perform home visits in case of emergency situations and to perform staff supervisory visits.

QUALIFICATIONS:

• Recent Home Health Experience required
• A minimum of 5 years of experience as a Home Health RN
• Recent intravenous administration and catheter management experience
• Excellent assessment skills
• Good communication, organizational, and customer service skills
• AXXESS software experience a plus.
• RN licensed in the state of Texas, with current license in good standing.
• Experience with Oasis documentation and Plan of Care
• Possess a current driver's license, reliable transportation, and proof of current and valid auto liability insurance.
