Current Clinic Openings Include: Live Oak, Westover Hills, Guilbeau, Kirby, DeLeon, and South Zarzamora. Conviva Care Centers, a subsidiary of Humana Inc., is seeking a primary care physician for our clinic in San Antonio Texas.
You have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinates of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused
primary care with a multidisciplinary approach.
A Primary Care opportunity on our team can offer you: - 100% Outpatient Schedule – Monday - Friday (8am to 5pm)
- Time with your patients – hour-long (initial) and typically, 40 to 45-minute-long appointments

Required Qualifications - Doctor of Medicine or Osteopathic Medicine
- Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
- Valid, unrestricted license in the state of desired employment (or willing to obtain)
- Current, unrestricted DEA license
